Making an Appointment
MAKE AN APPOINTMENT
To make an appointment with the LSUHSC-S Outpatient Psychiatry Clinic please call (318)813-2450 and an experienced hospital admissions tech will assist you Monday-Thursday 8 a.m.-4:30p.m. and Friday *8 a.m.-12:30p.m.
To better serve you we encourage you to arrive early for your appointment time to fill out the initial paperwork.
What to Bring for Your Clinic Appointment
Please bring the following:
- Picture identification
- All insurance/referral information
- Required co-payment/deposit
- LSU patient ID card.
- Medications - Prescription bottles and over-the counter medications
Non-Louisiana Residents: If you do not have a 3rd party payer (insurance, Medicare, Medicaid, etc.) and are unable to provide valid proof of Louisiana residency at the time of registration, you may be required to pay a minimum $250.00 Non-Louisiana Resident fee at each clinic visit.
